Documentation Assistance

We can help you with your I-693 Medical Examination and Vaccination form if you have been living in the United States for at least one year. 

You need to bring: 

  • Photo ID
  • I-94 Card
  • Employment Card
  • Immunization Card
  • Proof of Address
